The researchers at the University of Las Vegas, Las Vegas-based Dr. Joshua L. Fiducari, found that men taking Viagra for impotence had lower sexual satisfaction levels than those taking a placebo.
"This suggests that even the best treatments are not as good as the older drugs for erectile dysfunction," said study leader Dr. John C. Leucht, professor of urology at the University of Southern California and lead author of the study.
The results are published online April 1 in the journal Sexual Medicine.
The study is the first that shows the benefits of older drugs. Viagra is the only FDA-approved drug for erectile dysfunction that lasts 24 hours.
The drug's effectiveness is based on the theory that the drugs work in a similar fashion to treat the underlying causes of ED.
Viagra is also the only medication that was approved by the Food and Drug Administration in the United States for ED.
The researchers also found that patients taking Viagra had a lower rate of side effects from the drugs compared to those taking a placebo. Viagra also is approved for the treatment of impotence in men 18 and older.
Fiducari said the results may be because Viagra works much better in older men.
He said Viagra works for only a few minutes, so patients often wait at least four hours before taking a new medication.
It's not clear how effective the newer drugs are for Viagra, but experts say they can work for men with ED and women with ED and that they can work for those who need them.
"We know that older medications can be effective, but they are not as effective as Viagra, which is what we want to know," said Dr. Peter K. Dombrowski, an urologist at Northwestern Memorial Hospital in Chicago.
Dombrowski said older men who take Viagra don't have to take more drugs. They have to wait for a month to feel the effects of the drugs.
Men with erectile dysfunction take Viagra only when they are sexually excited. It does not work for older men who don't have the underlying condition causing ED.
If the drugs do work, they may help men whose ED has worsened, Dr. Kebede said.
Still, the study did not find that older treatments were better than the older drugs.
"It doesn't necessarily mean that the newer drugs are superior," said Dr. John R. Gersh, a urologist at Northwestern Memorial Hospital in Chicago and a co-author of the study.
Gersh noted that older medications are not as good as older drugs, which include a class of drugs called "PDE-5 inhibitors," and drugs such as Sildenafil, the generic name for Viagra.
Both Sildenafil and Viagra work in a similar fashion to treat ED in men.
In the study, men taking either one of the older drugs reported a reduction in their side effects compared to men taking a placebo.
Both drugs are available under the brand names Stendra and Vitaros, and are used to treat erectile dysfunction in men. A study of the older drugs in the U. S. found that the best drugs were Sildenafil and Vitaros, while the new drugs were Sildenafil Citrate, an older class of erectile dysfunction drugs.
In the study, Sildenafil, Vitaros and Viagra were all placebo-controlled, and in the study, the patients had to wait longer to see the effects of the drugs compared to men taking the older drugs. The differences in the two drugs may explain some of the differences in the rate of side effects.
Some of the more recent findings may have been more surprising, said Dr. Mark S. Bessette, director of urology at Brigham and Women's Hospital in Boston.
"The older drugs are probably not as good as the newer drugs," Bessette said.
Dr. Kebede said the older drugs may help men whose ED has worsened, but it was not clear how effective the newer drugs were for men with ED.
"There are probably still drugs to treat erectile dysfunction and they can help men whose ED has worsened, but they are not as effective as Viagra, which is what we want to know," he said.
Fiducari said there is still a small subset of men with ED who use Viagra.
